Your initial consultation is your time to meet your physician and our staff, ask questions and get the right answer to address your concerns. UltimA Medical Spa & Laser Center

Our office is located at 324 5th St., Suite 102, Lewiston, Idaho. (Map) Patients are seen by appointment, which can be booked by calling 208-746-2810, or contact us by the “request an appointment” or “request information” link.

If you must cancel, please provide us with at least 48 hours notice. No shows without calling in to cancel or reschedule will incur a fee of $60.00 for each hour of time scheduled.

Fees, Payment and Insurance

We make every effort to minimize your cost for appointments and treatment. All elective cosmetic procedures must be paid in advance before the procedure is performed. Some initial consultations may be covered under your medical insurance. However, Dr. Sandra Lotstein’s office will not bill for you, but we can provide the proper information that you may need so you may collect from your insurance. If your insurance requires further paperwork, we will bill you to send that information to them. You will always be contacted before billing so that you know what your insurance company is requesting. Our practice accepts cash, checks, most debit/credit cards and for your convenience there are other financial arrangements available.

Telephone calls

We encourage you to call or email with questions, compliments or complaints. The medical staff is trained to answer most calls or can forward your call to our physicians.

UltimA HIPAA Policy

UltimA maintains your health care information in a confidential manner. Your information will only be used to record your treatments, payment and health care operations.

Your rights as a patient of UltimA, a Medical Spa and Laser Center under the Health Insurance Portability and Accountability Act (HIPAA) include the following;

  1. You have the right to see and get copies of your record.
  2. You have the right to request that specific person(s) not see your record.
  3. You have the right to receive your medical information in a confidential fashion.
  4. You have the right to request that your medical records be amended.
  5. You have the right to request an accounting of everyone to whom the office reveals your medical information for purposes other than treatment, payment or office operation.
